We are seeking an experienced and motivated Registered Nurse Clinical Manager to lead our clinical team and support our continued growth. Position Summary The Home Health Clinical Manager (RN) is responsible for overseeing the day-to-day clinical operations of our home health agency. This leadership role ensures the delivery of exceptional patient care, regulatory compliance, quality outcomes, and staff development. The Clinical Manager will provide clinical supervision to nursing and interdisciplinary staff while collaborating with physicians, referral sources, patients, and caregivers. The ideal candidate is an experienced home health nurse leader with strong knowledge of Medicare Conditions of Participation, OASIS, care coordination, and quality improvement initiatives. Responsibilities Provide clinical leadership and supervision to nursing and interdisciplinary team members. Oversee patient admissions, care planning, case conferences, and care coordination activities. Ensure compliance with Medicare, state, federal, and accreditation requirements. Review and approve clinical documentation for accuracy, completeness, and regulatory compliance. Monitor quality measures, patient outcomes, and performance improvement initiatives. Conduct clinical audits and participate in agency quality assurance programs. Mentor, coach, and develop field clinicians to support clinical excellence. Collaborate with physicians and referral partners to ensure continuity of care. Participate in hiring, onboarding, training, and performance evaluations. Support census growth through exceptional clinical service and referral source satisfaction. Assist with on-call clinical support as needed. Required Qualifications Active Registered Nurse (RN) license in Massachusetts. Minimum of 3 years of home health experience. Minimum of 1 year of supervisory or management experience in home health, hospice, or community-based healthcare. Strong knowledge of Medicare regulations, Conditions of Participation (CoPs), and OASIS. Excellent leadership, communication, and organizational skills. Proficiency with electronic medical records and home health software. Valid driver's license and reliable transportation. Preferred Qualifications COS-C or HCS-O certification. Experience with Medicare reimbursement and quality reporting programs. Previous experience managing interdisciplinary home health teams.
