Job Description
Overview TruHorizon Homecare is seeking a compassionate, experienced, and motivated RN Clinical Supervisor to oversee clinical quality, client assessments, care planning, and caregiver supervision. This role is essential in ensuring our clients receive exceptional care while maintaining compliance with Georgia home care regulations and agency policies. The ideal candidate is passionate about serving seniors, supporting caregivers, and helping clients maintain their independence and quality of life at home. ResponsibilitiesClinical Oversight Conduct initial client assessments and ongoing reassessments Develop, implement, and update individualized care plans Monitor client health status and recommend appropriate services Ensure quality care is delivered according to agency standards Caregiver Supervision Conduct supervisory visits and competency evaluations Train, mentor, and support caregivers, CNAs, and PCAs Monitor caregiver performance and documentation Assist with caregiver education and continuing training Compliance & Quality Assurance Ensure compliance with Georgia home care regulations Maintain accurate client records and documentation Participate in quality assurance and risk management activities Assist during audits, surveys, and accreditation reviews Client & Family Support Serve as a clinical resource for clients and families Communicate with physicians, hospitals, case managers, and referral partners Address client concerns and help resolve care-related issues QualificationsRequired Active Georgia Registered Nurse (RN) License Minimum 2 years of nursing experience Strong clinical assessment skills Excellent communication and leadership abilities Valid driver's license and reliable transportation Ability to travel throughout Metro Atlanta and Northwest Georgia Preferred Home Care Experience Home Health Experience Hospice Experience Case Management Experience Supervisory Experience Compensation & Benefits Competitive Pay Flexible Schedule Supportive Leadership Team Career Growth Opportunities Meaningful Work with a Mission-Driven Organization Positive and Collaborative Work Environment About TruHorizon Homecare At TruHorizon Homecare, we believe everyone deserves compassionate, personalized care in the comfort of their own home.
