RN, Palliative Care Team Manager 3.6 3.6 out of 5 stars 275 N El Cielo Rd Ste C, Palm Springs, CA 92262 Hybrid work $50.04 - $61.04 an hour - Full-time Desert Oasis Healthcare 76 reviews $50.04 - $61.04 an hour - Full-time The Palliative Care Team Manager, primary responsibility is to supervise the Palliative Care interdisciplinary group (IDG) in the coordination and facilitation of Palliative Care services to DOHC members. Professional Duties Perform concurrent clinical review of patient plans of care, to identify clinical interventions required where appropriate, while transitioning patients to appropriate program and level of care. Oversee the IDG coordination of care to ensure appropriate utilization e.g. using Immediate Care vs ER, expedite referrals. Perform discharge planning for PC members who no longer meet eligibility criteria for the Palliative Care program. Perform clinical review to identify performance improvement and educational opportunities for IDG members. Collaborate with Quality Manager in enhancing dashboard for PC quality measures. Participate in Leadership meetings, Quality Assessment Performance Improvement meetings, In-Service Educational opportunities, and participate in the Administrative On-Call rotation. Produce team schedules ensuring patient allocations are fair and equitable amongst IDG members, allowing for acuity and location of patient with the goal of meeting efficient productivity expectations. Perform data collection for PC Referral Log, on a daily basis to ensure accuracy of information and reporting to dashboard. Collaborate with DOHC departments/programs to decrease unnecessary utilization and maximize positive patient outcomes. Validate Encounter Data timely, for monthly presentation to leadership. Following provided reports and timeframe guidelines from DOHC; perform analysis of HCC coding and identify members with outstanding codes not captured. Coordinate IDG to ensure members attend Senior Only Evaluation Clinic. Perform continuous audit of 5 Star Measures; delegating outstanding measures to IDG to coordinate. Collaborate with DOHC personnel re Care of the Adult measure, to ensure efficiencies of data collection. Oversees and manages cost containment. Performs other duties as assigned. Registered Nurse shall have a minimum of 1 year experience as a professional nurse within the last 3 years. A minimum of 2 years of supervisory or management experience. Demonstrates clear understanding of Medicare Conditions of Participation, CA Standards of Hospice Care and other federal and state regulations relating to hospice and palliative care. Demonstrates the ability to supervise and direct professional and administrative personnel. Able to multi task effectively. Proficient in computer use. Valid California Drivers license proof of automobile insurance.
