This position includes field-based client assessments, development and oversight of plans of care, supervision of Certified Nursing Assistants and homemakers, clinical documentation review, and coordination of home care services. The RN Clinical Supervisor reports directly to the Director of Nursing and works collaboratively with clients, families, caregivers, direct-care staff, healthcare providers, office personnel, and payer representatives. Schedule Full-time, 40 hours per week Monday through Friday Combination of field visits and office-based clinical responsibilities Occasional schedule adjustments may be necessary to accommodate client availability and required supervisory deadlines Participation in clinical coverage or after-hours consultation may be required according to the agency's coverage schedule Essential Responsibilities Conduct initial in-home assessments before services begin or within the timeframe permitted by applicable regulations. Complete client reassessments and supervisory visits at the frequency required by Rhode Island regulations, payer requirements, the client's condition, and agency policy. Develop, review, sign, and revise individualized plans of care and service task lists. Identify clients' personal-care, homemaking, safety, functional, and health-related needs. Establish measurable care objectives with participation from the client, representative, family, and care team when appropriate. Ensure assigned services are authorized, clinically appropriate, and consistent with the current plan of care. Conduct on-site supervision and performance monitoring of Certified Nursing Assistants and homemakers. Observe staff performance and verify competency, adherence to the plan of care, infection-control practices, client rights, safety requirements, and professional boundaries. Assign only duties that may legally and safely be performed by the assigned worker. Identify services or activities that fall outside a CNA's or homemaker's authorized scope and immediately address concerns. Provide coaching, corrective education, and competency validation as assigned by the Director of Nursing. Review CNA and homemaker documentation for completeness, accuracy, timeliness, and consistency with the plan of care. Verify that completed, refused, omitted, or unavailable services are documented appropriately. Monitor client records for changes in condition, missed visits, service discrepancies, incidents, hospitalizations, complaints, and other required follow-up. Communicate significant changes or concerns to the Director of Nursing and appropriate healthcare providers. Coordinate services with clients, families, providers, payer representatives, and other involved professionals. Participate in admission, transfer, hospitalization, resumption-of-care, and discharge processes. Provide client and caregiver teaching within the registered nurse's scope of practice. Complete all assessments, supervisory notes, plans of care, incident follow-up, and other clinical documentation in the agency's electronic medical record within established timeframes. Participate in clinical-record audits, quality-improvement activities, corrective-action monitoring, and regulatory readiness. Assist with staff orientation, in-service education, and clinical competency evaluations. Recognize and report suspected abuse, neglect, mistreatment, exploitation, unsafe conditions, and communicable diseases as required. Maintain client confidentiality and comply with HIPAA, agency policies, professional nursing standards, and applicable Rhode Island requirements. Perform other clinically appropriate duties assigned by the Director of Nursing. Qualifications Current and unrestricted Rhode Island Registered Nurse license. Home care, community health, case management, long-term care, or supervisory nursing experience strongly preferred. Current CPR/BLS certification. Knowledge of nursing assessment, care planning, delegation, CNA scope of practice, and clinical documentation. Strong organizational, communication, teaching, and time-management skills. Ability to independently manage an assigned caseload and required supervisory deadlines. Ability to use an electronic medical record and other computerized systems. Valid driver's license, reliable transportation, and required automobile insurance. Ability to travel throughout assigned areas of Rhode Island. Bilingual English/Spanish strongly preferred. Experience working with adult, geriatric, pediatric is preferred. Pre-Employment Requirements The selected candidate must successfully complete all required credential verification, reference checks, criminal background screening, health screening, immunization review, orientation, and competency requirements before independently providing services. Physical and Travel Requirements The position requires regular travel to clients' homes, the ability to enter residences with varying accessibility conditions, and the ability to perform nursing assessments and observe direct-care staff safely.
