Nealy Pierce is conducting an exclusive search for a Director of Care Coordination (RN) who will oversee Case Management, Utilization Review, discharge planning, Social Work and Palliative Care coordination for a nonprofit 700+ bed health system.

System-Wide Impact The Director will:
Lead approximately 65 professionals across two hospitals Manage two Case Management Managers and a Utilization Review Manager Own length-of-stay and discharge-planning performance Serve as the escalation point for complex placements Strengthen relationships with SNFs, rehabilitation facilities and home health providers Partner with physicians and executives on patient-flow strategy Translate departmental data into actionable performance improvements Reasons to Consider It Broad responsibility across the full care continuum Direct reporting relationship to the Vice President of Medical Affairs Mature department with an established management structure Well planned transition from the current Director who will be retiring Up to $170,000 salary with flexibility for exceptional experience Full relocation assistance Generous front-loaded PTO and full benefits Leadership Profile The organization is seeking a leader with five to seven years of acute care experience, including at least three years in Case Management or Care Coordination leadership. Experience within a large hospital, multi-hospital system or similarly complex environment is important. RN Active License. Master's degree required (if not in Nursing, must hold a BSN). CCM certification is a plus. Remain Close to the Lowcountry Take on greater system responsibility while staying within the coastal Southeast.
