  • Registered Nurse (RN) - Clinical Manager
  • to oversee clinical operations and patient care services in our home health division. This is your opportunity to lead with compassion, drive excellence, and be part of a team that treats each other like family.
  • Key Responsibilities:
  • + Provide daily clinical oversight of patient care services.
+ Coordinate referrals and admissions with internal and external sources. + Evaluate patient needs and agency capabilities before admission. + Supervise care planning, partner assignments, and care delivery. + Ensure compliance with regulatory standards and agency policies. + Mentor and coach clinical staff to promote morale and retention. + Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.

  • + Active, unrestricted RN license in the state of practice. + Minimum of two years RN experience (preferred) + At least one year of Home Health experience (preferred) + Current CPR certification.
