About This Role:
Be a Steady Guide for Patients & Families Provide clarity, compassion, and support during end-of-life care- when it matters most. Lead the Plan of Care Ensure every patient's care is aligned with their goals, focused on comfort, dignity, and quality of life. Collaborate Across the Care Team Work closely with physicians, social workers, chaplains, aides, and caregivers to deliver holistic hospice care. Own the Clinical Picture You are the hub
