•High Plains Senior Care is seeking an experienced RN Clinical Manager for our Lubbock office. The ideal candidate is highly organized, possesses strong leadership skills, and has prior management experience. Home Health experience is required, and familiarity with Homecare Homebase (HCHB) is preferred. Position Details:
Monday-Friday schedule in an office setting. Paid holidays. Competitive salary with opportunities for additional earnings.
Key Responsibilities:
Deliver skilled nursing services under the direction of the
DON/ADON
Monitor, document, and report patients' physical and mental conditions Complete and maintain accurate clinical and progress notes Communicate changes in patient condition to physicians and appropriate team members Review OASIS documentation for accuracy and compliance Manage workflows within Homecare Homebase (HCHB) Conduct daily verifications and review OASIS warnings Approve interim orders and maintain the patient hold list Prepare for and lead weekly case conferences Review and finalize 485s; ensure timely OASIS lock Coordinate scheduling, PRN assignments, and admissions Handle triage calls, physician communication, and insurance authorizations Oversee team compliance and accountability Monitor and respond to emails promptly Participate in staff meetings and patient care discussions
