Overview Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN) - Charge Nurse, where your expertise will lead compassionate patient care and operational excellence. In this vital role, you will oversee nursing staff, coordinate patient assessments, and ensure the delivery of high-quality medical services across various care settings. Your leadership will foster a positive environment focused on safety, efficiency, and exceptional patient outcomes. We are seeking energetic, detail-oriented professionals committed to making a difference in the lives of our diverse patient population. Responsibilities Supervise and coordinate nursing staff to deliver comprehensive patient care in accordance with facility policies and healthcare standards. Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews utilizing EMR. Administer medications, injections, IV infusions, and perform catheterizations while adhering to HIPAA regulations and sterile techniques. Lead triage activities, evaluate urgent patient needs, and prioritize care for patients with acute pain or infectious diseases in emergency or inpatient settings. Oversee documentation of medical records, ensuring accuracy in
ICD-9/ICD-10
coding and compliance with healthcare regulations. Coordinate discharge planning and collaborate with case management teams to facilitate smooth transitions of care across post-acute or home care environments. Provide training and mentorship to nursing staff on topics such as wound care, dementia care including Alzheimer's and memory care, ventilator management, and advanced life support procedures. Experience Valid LVN license with current state certification required; additional RN licensure is a plus. • Demonstrated proficiency with EMR/EHR systems like Pointclick Care for medical documentation. Extensive background in diverse clinical settings including long-term care, assisted living, hospice care, outpatient clinics, or home & community health. Familiarity with specialized areas such as geriatrics including dementia and Alzheimer's care, infectious disease management, and pain management protocols. Strong knowledge of anatomy, physiology, diagnostic evaluation techniques and medication administration. Experience working with people with disabilities or developmental disabilities is desirable; familiarity with CPT coding for procedures is advantageous.
