Resident Care Director (LVN/RN) — Run the care in an 85-unit AL & Memory Care community Sundale Senior Living | The Woodlands, TX Full-time

It might be you. The one the building turns to when a resident goes sideways at 2 p.m. and the family is in the lobby at 2:15. The one who reads a fax, sees the order is wrong, and calls the doctor before anyone else notices. Whose med techs pass meds the way they were trained because someone checks. Whose caregivers report a change in condition the same shift because they were taught what to look for and what happens if they don't. Someone who has run a care department before, and is tired of doing it with no structure, no backup, and a corporate office three states away. If that's you, keep reading. What you'd walk into 85 units: 57 assisted living, 28 secured memory care. Family-owned, two communities in Texas, owners you'll actually talk to. A structure that's already built. You'll have an AL Care Coordinator running the AL schedule, call-outs and assignments for you. The Memory Care Director runs a separate department. You don't run the schedule. You run the care. One system for everything. August Health for assessments, service plans, eMAR, care tracking and documentation. No paper charts. No three logins. Clear authority. Every med tech in this building passes meds because you delegated them. You decide who's competent. You pull anyone who isn't. Nobody overrides that. What the job actually is Admission, change-of-condition and annual assessments; service plans written, current and signed. The medication program on every cart — new orders in August Health within 24 hours, pharmacy follow-up, cart and fridge audits, quarterly narcotic disposal, med pass audits on every med tech you delegate. Change-of-condition response, same day: assess, call the physician, get the order, document, call the family. Fall interventions in place immediately, checked for effectiveness within 72 hours. Weekly wound rounds, labs, x-rays, home health and hospice coordination, vaccine clinics. Care conferences and family communication — you're the clinical voice families trust. Coaching and training the care team; building the AL Care Coordinator into a lead who can run without you.
Survey readiness every day:
chart audits, compliance audits, and documentation that holds up under HHSC. On-call rotation for care issues and incidents, callback within 15 minutes. Manager-on-duty rotation. Support for tours and move-in assessments. What this job is not Not a scheduling job. Not a desk job — you round every day and every resident knows your name. Not a place where a problem waits for Monday. Non-negotiables Active Texas LVN or RN license in good standing. 3+ years leading a care team in assisted living, memory care or long-term care, at a community of 40+ units. Hands-on eMAR/EHR experience. August Health experience is a plus. Working knowledge of Texas HHSC Chapter 553 and the backbone to say "not until the assessment is done" when sales wants a move-in tomorrow. Commute of 30 minutes or less at morning rush hour, or willing to relocate. On-call doesn't work from an hour away. Nice to have Certified Dementia Practitioner. Current CPR/First Aid.
