Job Description

The Clinical Manager (Registered Nurse or Physical Therapist) is responsible for coordinating and supervising clinical services to ensure the delivery of high-quality home care to patients. This role oversees clinical staff, ensures compliance with regulatory and payer requirements, and supports the coordination of patient care services. The Clinical Manager serves as a clinical resource to the care team and works closely with physicians, third-party sources, and interdisciplinary team members to ensure appropriate authorization, reporting, and documentation of services.
KEY JOB FUNCTION
Provides leadership and supervision to assigned clinical staff and conducts case conferences, team meetings, and in-service trainings as needed. Coordinates home care services with physicians, interdisciplinary team members, and third-party payor sources to ensure appropriate patient care and required authorizations. Oversees the assignment, scheduling, and supervision of clinical staff, including on-call coverage, and maintains records of case assignments. Organizes and facilitates patient care conferences to promote coordination of services and positive patient outcomes. Reviews medical records regularly and conducts periodic clinical record audits to ensure quality care and proper documentation. Investigates adverse events or incidents and participates in quality improvement activities to enhance patient care and compliance.
QUALIFICATIONS
Registered Nurse (RN) / Physical Therapist (PT) with current license to practice professional nursing in the State of Virginia. Three to five years of clinical experience with emphasis in Home Health. Excellent understanding of performance improvement, quality assurance, and utilization management. Knowledge of state, federal and Joint Commission Excellent written and communication skills
