This role ensures coordination of care, regulatory compliance, and high-quality clinical outcomes. The position combines case management responsibilities with clinical supervision and interdisciplinary coordination to promote continuity of care and patient safety. Essential Duties & Responsibilities/RN Case Management Responsibilities Conduct 30-day head to toe reassessments and ongoing patient evaluations. Monitor patient progress and communicate significant changes in condition to physicians and appropriate team members. Develop, update, and implement individualized Plans of Care in collaboration with physicians. Provide patient and caregiver education on diagnoses, treatments, medications, and disease management. Ensure all clinical documentation is complete, accurate, and compliant with federal and state regulations. Facilitate case conferences and interdisciplinary communication to support coordinated care. Clinical Coordination Responsibilities Provide supervision and clinical support to RNs, LPNs, and aides. Review clinical documentation for quality assurance and regulatory compliance. Support staffing coordination to ensure appropriate coverage and continuity of care. Participate in Quality Assurance and Performance Improvement (QAPI) activities. Ensure adherence to Medicare, Medicaid, and applicable state regulations. Assist with audit preparation, internal reviews, and regulatory surveys. Investigate, document, and follow up on clinical incidents or concerns as needed. Qualifications Current, unrestricted Registered Nurse (RN) license in the state of practice. Minimum of 2-3 years of nursing experience (home health, private duty, or case management preferred). Strong knowledge of Medicare, Medicaid, and state regulatory requirements. Experience with care coordination and interdisciplinary collaboration. Strong leadership, organizational, and communication skills. Proficiency in electronic medical record (EMR) systems Current CPR certification ICU and PACU experience preferred Key Competencies Strong clinical judgment and critical thinking skills Regulatory compliance knowledge Effective care coordination and time management Leadership and team collaboration Professional communication and documentation accuracy
