Stay Connected:
Communicate regularly with residents and their families about care plans, therapies (PT/OT), and treatments. Your updates make a big difference. Be a

Provide Clarity and Comfort:
Answer residents' questions about their care, treatments, and conditions with empathy and confidence—they trust you.
Make Smart Moves:
Monitor health changes and adjust care plans as needed—you're always one step ahead.
Own Your Day:
Prioritize and organize care for new, current, and discharging residents—you're the go-to for knowing what needs to happen and when.
Keep It Accurate:
Document clearly and thoroughly—you understand the importance of good records and great communication. Collaborate for
Success:
Work closely with healthcare providers, financial services, and insurance partners to ensure residents receive the best care—efficiently and affordably. Guide the
Journey:
Oversee all admissions and discharges and coordinate smooth, respectful transitions of care.
Support Referrals:
Help residents and families find the right next step, whether they're coming to us or moving forward in their care journey.

Role:
Bachelor's degree in Nursing or Social Work (RN license; LVN/LPN accepted) 2+ years of clinical experience preferred Strong understanding of Medicare, Medi-Cal, and Medicaid Intermediate computer skills Excellent time management and communication skills Positive attitude, problem-solving mindset, and genuine care for others Certification as a Case Manager (CCM) a plus

$90,000-$120,000 depending on experience and education.
