with a passion for geriatric care? One of our top clients is seeking a highly skilled RN Case Manager, for a premier luxury facility dedicated to providing exceptional care for our senior residents, is seeking a qualified RN Case Manager to join our dedicated team. In this vital role, you will play a crucial part in coordinating and overseeing the care plans of our residents, ensuring they receive the highest quality of life possible.
Responsibilities:
Conduct comprehensive assessments of residents' physical, emotional, and social needs, working closely with residents, families, and other healthcare professionals. Develop and implement individualized care plans, utilizing interdisciplinary team collaboration to address all aspects of resident well-being. Monitor and track resident progress, adjusting care plans as needed. Advocate for residents' rights and ensure their autonomy and dignity are respected. Provide education and support to residents and families on various aspects of aging and healthcare. Manage complex healthcare needs, coordinating with physicians, therapists, and other specialists. Maintain accurate and detailed documentation of residents' care. Foster a positive and collaborative working environment within the interdisciplinary team.
Qualifications:
Registered Nurse lpn OR (RN)
WISCONSIN
license with at least 1 years of experience in geriatric nursing, preferably in a long-term care setting. Strong clinical skills and knowledge of geriatric healthcare needs. Excellent communication, interpersonal, and collaboration skills. Ability to prioritize and manage multiple tasks effectively in a fast-paced environment. Strong organizational and time management skills. Compassionate and patient-centered approach to care. Proficient in electronic medical records systems.
