This is a dynamic, hybrid leadership role (approximately 80% clinical operations/auditing and 20% direct field coverage). The ideal candidate is a highly organized operational leader who is also clinically proficient and willing to step into the field to provide direct, complex care when staffing needs arise. This role serves as the clinical safeguard for the agency, ensuring all operations meet the highest standards of safety and strict compliance with Wisconsin Administrative Code for skilled nursing agencies. Key Responsibilities and Duties (Clinical, Operational & Compliance) 1. Clinical Operations, Staffing & Compliance (Office - 80%)
Plan of Care Management:
Perform initial clinical evaluations and develop, initiate, and revise written Plans of Care (POCs) for high-acuity pediatric patients, ensuring all physician/PCP orders are accurately incorporated.
Staff Management & Scheduling:
Oversee the daily scheduling of field RNs and LPNs, ensuring appropriate clinical matching for complex cases (e.g., ventilators, tracheostomies, TPN).
Regulatory Auditing:
Conduct rigorous monthly and quarterly audits of patient charts. Ensure all documentation, clinical notes (signed within 7 days), POC renewals, and physician orders are strictly compliant with Wisconsin state regulations (DHS 133) to maintain impeccable agency licensure standing.
Care Team Supervision:
Conduct regular field supervisory visits to evaluate the clinical competency of field nurses, providing ongoing training, skills sign-offs, and annual evaluations for acute pediatric interventions.
Physician Coordination:
Serve as the primary clinical liaison between the agency, field nurses, and physicians, promptly communicating any changes in a patient's condition and managing order renewals every 60 days. 2. Direct Patient Care & Field Coverage (Field - 20%)
Emergency Shift Coverage:
Act as the ultimate clinical backstop. In the event of a field nurse call-out, step in to provide direct, hands-on skilled pediatric nursing care in the patient's home.
High-Acuity Interventions:
Competently execute complex clinical tasks in the field, including but not limited to ventilator and tracheostomy care, oxygen management, TPN infusions, and emergency seizure management.
Patient Re-evaluations:
Perform hands-on physical assessments and functional evaluations in the home to adjust the POC as a child's medical needs evolve.
Required Qualifications:
Licensure:
Current, unencumbered Registered Nurse (RN) license in the State of Wisconsin.
Experience:
Minimum of 3-5 years of clinical nursing experience, 1+ experience in home health, and a plus for a background in high-acuity pediatrics (PICU, NICU, or pediatric private duty nursing).
Supervisory Experience:
Prior experience in a clinical management or supervisory role overseeing RNs and LPNs.
Knowledge:
Comprehensive knowledge of federal and Wisconsin state rules, regulations, and procedures pertaining to Skilled Home Health Agencies. Pediatric ventilator and tracheostomy care (Will Provide Certification if Necessary) G-tube feeding and TPN infusion (Required) Respiratory monitoring and oxygen management Seizure management and emergency response Medication administration and injections
Other:
Current CPR/BLS certification (PALS preferred), valid driver's license, and reliable transportation with current auto insurance.
