AKA:
Your Superpowers)Lead Medication Management:
Oversee and inspect medication administration and documentation. Investigate and resolve all unadministered medication events (PCC code 13). Audit medication storage and carts monthly (ensuring correct dating, expiration, labeling, etc.). Monitor medication refrigerator logs and ensure temperature compliance (36-46°F). Delegate and supervise medication passers; oversee narcotic counts per policy. Evaluate resident capability for self-administration and ensure all required documentation is in place. Support compliance with HDG medication error policies and ensure corrective actions are followed.Support Clinical Assessments & Resident Documentation:
Participate in assessments and updates per DHS 83.35, ensuring medical needs and medication goals are clearly defined. Ensure care level documentation and service plans reflect changes in condition. Complete quarterly psychotropic medication reviews in compliance with state regulations. Maintain accurate records for bed rail usage and associated documentation.Champion Compliance, Quality & Risk Management:
Follow up on clinical quality action plans and Plans of Correction. Assist with determining appropriate residency placement using assessments and RYG grid. Partner with the Executive Director in preparing State Reportable event documentation. Monitor incidents involving resident injuries and verify follow-up actions. Ensure required annual documents (e.g., H&Ps, TB screenings, med lists, advance directives) are current and uploaded in PCC. Verify legal documentation is accurately reflected in the resident record. Ensure timely completion of vaccination clinics and communicable disease screenings for residents and staff. Educate & Empower theCare Team:
Lead staff education sessions including: Standard precautions First aid and choking protocols Fire safety procedures Serve as a clinical resource and mentor for LPNs and community nurses. Support care staff with resident condition changes or clinical questions. Be aHands-On Clinical Leader:
Step in as a floor nurse as needed, especially during emergencies or staffing gaps. Provide support with direct care and medication administration if needed. What You Bring to the Table (Besides Clinical Skill)Education & Licensure:
Current RN license in the state of employment required. CPR certification and strong knowledge of infection control best practices requiredExperience & Expertise:
2+ years of clinical nursing experience, preferably in an assisted living or long-term care setting. Familiarity with DHS 83 regulations and the service plan development process is a plus.Skills & Competencies:
Excellent documentation, auditing, and regulatory compliance skills. Strong problem-solving ability and attention to detail. Skilled in coaching and collaborating with multidisciplinary teams.Professional Traits:
