REHAB REFERRAL COORDINATOR

Job Description The Rehab/Prospective Payment System (PPS) Referral Coordinator is a professional Registered Nurse who reviews patient referrals to the Rehab Unit, both in-house and from outside facilities. Coordinates referrals to promote timely admission. Assists with data collection for the QI process. Identifies and assists with Rehab specific education needs. Experience/Qualifications Knowledge of patient care standards and practices is normally acquired through completion of a nursing program. Bachelor's degree in nursing preferred. A minimum of two years' related experience. Possesses an active Wisconsin RN nursing license. Working knowledge of IRF PAI and
FIM. FIM
(Functional Improvement Measurement) credentialed. PPS certification desired. Current knowledge of Rehab CMS rules and regulations. Interpersonal skills necessary to effectively communicate with physicians, nursing personnel and team members.
