Certified Dietary Managers (CDM) or Experienced Diet Techs are encouraged to apply! High School graduate or equivalent; Basic computer skills; Strong communication skills; Experience in a clinical and/or dietary setting is preferred but not required.WHO ARE WE LOOKING FOR?
The Rehabilitation Center at Hartsfield Village is a dynamic Post-Acute care setting in Munster, Indiana. We are seeking an enthusiastic candidate looking for opportunity for growth and advancement. This individual will work under the general supervision of a Registered Dietitian and the Director of Nursing in support of both the Clinical and Hospitality and Nutrition Services departments.WHAT YOU WILL DO
Conduct patient interview and assessments upon admission and as scheduled thereafter in order to assess and collect food preferences and/or menu selection. Provide education to both patients and/or staff related to diet orders and preferences. Performs limited related dietary duties such as: recording allergies and preferences, preparing nourishment lists, etc. Coordinate with the Interdisciplinary Team (IDT). Updates patient medical record (EMR) and Meal Tracker software with diet information, patient preferences, nourishments, etc. Maintains records of all phone calls and completes all phone messages procedures according to established practices. Responsible for various record keeping and clerical duties (ie: menu distribution, etc.). Maintains records completely and accurately. Proactively performs duties in a timely manner, accurately and consistently. Should have a working understanding of regulatory guidance for Long-Term Care settings (IDOH). Communicates with supervisor concerns and reports discrepancies. Why Join Our Team?
