Glendale, AZ Pay:

Horizon Post Acute is looking for a passionate Occupational Therapist (OT) to join our growing in-house therapy team . What You'll Do As an Occupational Therapist, you will: Evaluate residents and develop individualized occupational therapy treatment plans Provide skilled OT interventions to improve functional independence Help residents improve ADLs, mobility, coordination, strength, cognition, and safety Develop meaningful goals based on each resident's abilities and needs Monitor progress and modify treatment plans as appropriate Collaborate with COTAs, PT/PTAs, nursing, physicians, and other members of the interdisciplinary team Educate residents, families, and caregivers Complete accurate and timely clinical documentation Participate in care planning and interdisciplinary meetings as needed What We're Looking For Current Arizona Occupational Therapy license New graduates are welcome to apply Strong clinical and communication skills Ability to work collaboratively with an interdisciplinary team Compassionate, dependable, and patient-centered approach Someone who takes pride in helping residents achieve meaningful goals What We Offer $40•55/hour based on experience Ask about our Sign-On Bonus!
