This role collaborates closely with physicians, nursing staff, rehabilitation professionals, patients, and families to develop effective treatment plans and achieve optimal outcomes. Occupational Therapist Responsibilities Evaluate patients within required regulatory timeframes following physician referral Develop individualized treatment plans and establish measurable goals Provide occupational therapy services in accordance with physician orders and professional standards Assess patients' abilities to perform activities of daily living (ADLs) Supervise Occupational Therapy Assistants (COTAs), aides, and students as appropriate Collaborate with physicians, nursing staff, and interdisciplinary team members regarding patient progress and care plans Participate in care conferences, rehab meetings, and discharge planning Educate patients, families, caregivers, and staff on treatment plans and therapy techniques Recommend adaptive equipment, assistive devices, and durable medical equipment as needed Complete accurate and timely documentation in compliance with federal, state, and facility guidelines Maintain therapy equipment and report maintenance or repair needs Promote a positive, compassionate, and patient-focused rehabilitation environment Qualifications Current and active California Occupational Therapist License required Bachelor's degree in Occupational Therapy required Master's or Doctorate in Occupational Therapy preferred Skilled nursing, rehabilitation, post-acute, or long-term care experience preferred Strong clinical assessment and treatment planning skills Excellent communication and interpersonal abilities Experience with PointClickCare and Casamba preferred Ability to work effectively in a fast-paced, team-oriented environment Commitment to delivering exceptional patient-centered care About Citrus Heights Post Acute At Citrus Heights Post Acute, we are committed to helping our patients achieve their highest level of independence and recovery.
