Overview We are seeking a compassionate and motivated Occupational Therapist (OT) to join a 126-bed skilled nursing facility in Fremont, California . This is a great opportunity for an OT who enjoys a fast-paced clinical environment, meaningful patient interaction, and helping individuals regain their independence following significant medical events. The patient population includes Kaiser patients, short-term rehabilitation patients, hip and knee replacement patients, and individuals recovering from neurological conditions . Why Join This Team? $60•65/hour for candidates with 1+ years of experience New graduates are welcome to apply New graduate compensation will be determined based on experience level, qualifications, and clinical background after individual review Permanent, full-time opportunity 35 hours per week 5 days per week Weekend rotation 126-bed skilled nursing facility Diverse short-term rehabilitation patient population Exposure to hip and knee replacement recovery Opportunity to work with patients recovering from neurological conditions Experience with Kaiser patient populations Collaborative interdisciplinary environment Opportunity to strengthen your clinical skills while making a measurable difference in patients' recovery Responsibilities Perform comprehensive occupational therapy evaluations and assessments Develop individualized treatment plans based on each patient's functional needs and goals Provide skilled OT interventions to improve independence with activities of daily living and functional mobility Treat patients recovering from orthopedic procedures, including hip and knee replacements Provide therapy for patients recovering from neurological conditions and other complex medical diagnoses Monitor patient progress and modify treatment plans as clinically appropriate Educate patients and caregivers on therapeutic techniques, safety strategies, and continued progress toward independence Collaborate with physical therapists, nursing staff, physicians, and other members of the interdisciplinary team Participate in care planning and discharge planning Maintain accurate, timely, and compliant clinical documentation Communicate effectively with patients, families, caregivers, and other healthcare professionals Follow applicable clinical standards, facility policies, and regulatory requirements Qualifications Graduate of an accredited Occupational Therapy program Current California Occupational Therapist license or eligibility for licensure Strong clinical reasoning and communication skills Commitment to patient-centered care Ability to work effectively within an interdisciplinary team Strong organizational and documentation skills SNF or rehabilitation experience is a plus New graduates are welcome and encouraged to apply Compensation for New Graduates The $60•65/hour range is intended for Occupational Therapists with 1+ years of professional experience . New graduates are absolutely encouraged to apply. Their compensation will be individually reviewed and aligned with their level of experience, qualifications, and clinical background . Final compensation for a new graduate will be determined following the candidate review process.
