We are seeking experienced, dependable Occupational Therapists who are passionate about helping patients regain independence and improve their quality of life. Responsibilities Perform initial occupational therapy evaluations in patients' homes. Develop individualized treatment plans based on physician orders and patient needs. Provide skilled occupational therapy services to improve activities of daily living (ADLs), upper extremity function, cognition, home safety, and overall independence. Assess patients for adaptive equipment and recommend appropriate modifications to improve safety and function. Educate patients and caregivers on home safety, fall prevention, energy conservation, and adaptive techniques. Coordinate care with physicians, nurses, physical therapists, speech therapists, and other members of the healthcare team. Complete accurate and timely documentation in the client's EMR while maintaining Medicare and agency compliance. Qualifications Current California Occupational Therapist (OT) license in good standing. Minimum 1 year of Home Health experience preferred. Current CPR certification. Valid driver's license, reliable transportation, and auto insurance. Strong clinical judgment, communication, and documentation skills. Ability to work independently while managing a flexible schedule. Experience with electronic medical records (EMR) preferred.
