Occupational Therapist (Palmdale) Home Health Omega Home Health Care Services Inc Palmdale, CA Job Details Per diem 10 days ago Qualifications In-home healthcare experience Occupational Therapy License Full Job Description We are seeking a compassionate and skilled Occupational Therapist to join our home health team on a per diem basis. This role involves providing rehabilitative services to patients in their homes to help them regain independence and improve quality of life. Candidates must be licensed in California and experienced in home health or related settings.
Key Responsibilities:
Conduct comprehensive patient evaluations and develop individualized treatment plans Deliver occupational therapy services in patients' homes in accordance with physician orders and company protocols Instruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skills Monitor patient progress and modify treatment plans as needed Collaborate with multidisciplinary team including nurses, physical therapists, and social workers Maintain accurate and timely documentation in compliance with California state and federal regulations Ensure patient safety and adherence to infection control procedures in home environments
Qualifications:
Current and valid Occupational Therapist license in the state of California Graduate of an accredited Occupational Therapy program Minimum of 1 year experience in a home health or similar setting preferred Valid driver's license and reliable transportation Strong interpersonal, communication, and organizational skills Proficiency with electronic medical records (EMR) systems is a plus
Working Conditions:
Flexible, per diem schedule based on patient needs and therapist availability Travel within a designated geographic region required This position is paid by the visit. Actual pay rates vary depending on factors including type of visit, type of experience, location, and years of experience.
