This position offers an exciting opportunity to work across diverse settings, including inpatient units, outpatient clinics, schools, home health, and skilled nursing facilities. Your expertise will help individuals of all ages—pediatrics to geriatrics—achieve their functional goals and improve daily living skills. We are dedicated to fostering a collaborative environment that values innovation, compassion, and professional growth. Duties Assess patients' physical, cognitive, and emotional abilities to develop tailored treatment plans(Plan of Care) that promote optimal functioning. Assist patients with Activities of Daily Living (ADLs), such as dressing, grooming, and mobility, to support independence. Conduct comprehensive evaluations and assessments of patients' conditions, abilities, and functional limitations Educate patients and families on proper use of equipment and techniques to enhance their progress Maintain accurate and complete patient documentation Provide hands-on therapy and rehabilitation services in the home environment Develop and implement individualized treatment plans and goals to meet patients' needs and help them reach their maximum level of independence Engage in interdisciplinary teamwork to deliver comprehensive care. Ability to handle physically demanding tasks and work effectively in various home environments Requirements Valid occupational therapy license in the relevant jurisdiction. Experience in home health or community-based setting Ability to work independently and manage a flexible schedule and caseload of patients Experience in educating and training caregivers and family members. Strong clinical skills and knowledge of occupational therapy practices and techniques Commitment to providing patient-centered care and making a positive impact in patients' lives

$65.00 - $95.00 per hour
