$60/hour- 65/hour
Schedule:
All Shifts Available (Full-Time, Part-Time, PRN) Help Residents Regain Independence Through Purposeful Care. Sierra Valley Rehab Center in Porterville, CA is a skilled nursing and rehabilitation facility committed to delivering high-quality, resident-centered care. We are currently seeking a licensed and compassionate Occupational Therapist (OT) to join our therapy team.Position Summary:
As an Occupational Therapist, you will evaluate, plan, and provide therapeutic interventions for residents to improve their ability to perform daily activities. You will work closely with interdisciplinary teams to deliver personalized care plans and support recovery goals in a post-acute care environment.Key Responsibilities:
- Evaluate patients within 24 hours of physician referral or document cause for delay
- Develop and implement individualized treatment plans
- Provide direct therapy services and document outcomes in accordance with facility, state, and federal requirements
- Supervise Occupational Therapy Assistants (COTAs) and aides as per state guidelines
- Educate residents, families, and caregivers on therapy goals and home programs
- Participate in discharge planning, staff training, and in-service programs
- Attend care conferences and communicate patient progress to the interdisciplinary team
- Recommend adaptive equipment and modifications to support functional independence
- Maintain therapy equipment and report any maintenance needs
Qualifications:
- Bachelor's, Master's, or Doctorate in Occupational Therapy from an accredited program
- Current California OT license in good standing
- CPR certification (or obtained upon hire)
- Experience in skilled nursing or post-acute care preferred
- Proficiency in EMR systems such as Casamba or PointClickCare
- Strong clinical, organizational, and communication skills
Physical Requirements:
- Ability to assist residents in therapeutic movements and mobility tasks
- Frequent standing, walking, pushing, and lifting
- Able to lift 50+ lbs and perform physical care duties as needed Why Sierra Valley Rehab Center?
