Per Diem Job Description:
We are currently seeking a compassionate and experienced Occupational Therapist (OT) to join our team as an independent contractor in a home health setting. This role offers the opportunity to work one-on-one with patients in their homes, helping them regain independence and perform daily living activities with confidence. You will collaborate with a multidisciplinary team to deliver high-quality, goal-oriented therapy, while enjoying the flexibility of creating your own schedule and managing your own caseload.
Key Responsibilities:
Evaluate patients' functional abilities and develop customized treatment plans. Implement therapeutic activities to improve ADLs, motor skills, and cognitive function. Recommend adaptive equipment and provide training on its use. Educate patients and caregivers to support ongoing rehabilitation and safety in the home. Document patient progress and modify treatment plans as needed. Maintain timely and accurate clinical documentation in accordance with agency and regulatory standards.
Credential & Experience Requirements:
Active Occupational Therapist license in the state of California. Graduate of an accredited Occupational Therapy program. Minimum 1 year of clinical experience in home health strongly preferred. Current CPR/BLS certification. Valid driver's license, reliable transportation, and current auto insurance. Strong documentation skills and familiarity with EMR platforms. Excellent communication, time management, and problem-solving abilities.
