Location:
Riverside Central and East As a Home Health OT, you will: Provide skilled occupational therapy services in accordance with physician orders and the patient's individualized Plan of Care (POC). Evaluate patients' functional abilities and assist the physician and care team in determining appropriate treatment goals and interventions. Contribute to the development and ongoing updates of the patient's Plan of Care based on functional needs and progress. Assess the home environment and recommend appropriate modifications, equipment, or strategies to promote safety, independence, and quality of life. Monitor patient progress and communicate relevant changes and recommendations with the physician, case manager, patient, and family/caregivers. Educate and collaborate with patients, families, and LORIAN team members regarding therapy programs, techniques, and strategies to support continued progress. Maintain accurate, timely clinical documentation in accordance with LORIAN policies and procedures, while participating in continuing education and in-service programs. Required Qualifications Graduate of an occupational therapy school accredited by the Council of Medical Education of the AMA in collaboration with the American Association of Occupational Therapists and/or by the State Department of Education in the specific state where employed. Registered by the Occupational Therapist Association and licensed in the state of practice if the state has registered practices. Valid driver's license and insured personal vehicle (per Lorian requirements). Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting. Possesses and maintains current CPR certification. This position is based on a pay per visit compensation model. Pay per visit ranges from $50 to $120 visit commensurate with experience and depending on type of visit.
