Home Health Occupational Therapist (OT) Per Diem | San Mateo County — Dedicated Territory Assigned We are currently looking for a compassionate, organized, and team-oriented individual to fill the role of Occupational Therapist (OT) position for the SF - Peninsula area. For over 20 years, we've served the greater Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. We are hiring Occupational Therapists for Peninsula teams. You will be assigned to one dedicated territory — San Mateo County — so you can build familiarity with your patients and your area without being stretched across both.

Your caseload stays within a manageable radius in either SF or the Peninsula — no crossing bridges or unpredictable commutes. SF patients are spread across neighborhoods like the Sunset, Richmond, and Mission; Peninsula patients run from Daly City down through Redwood City.

Compensation & Benefits Base Salary:
$125,000 - $150,000 per year (based on experience)
Performance Bonus:
Up to 25% of base salary tied to quality metrics and clinical outcomes
Total Earning Potential:
$156,250 - $187,500 per year

Paid time off, vehicle reimbursement options, and company-provided devices (iPad) Your Role Provide skilled occupational therapy services focused on ADLs, adaptive equipment, and functional independence Develop, implement, and manage individualized plans of care based on physician orders Report changes in patient condition and collaborate with physicians, case managers, and the Director of Nursing Maintain accurate, timely clinical records using AI-supported documentation tools Requirements Current, active California Occupational Therapist license (required) Minimum of 1 year of home health experience strongly preferred Valid California driver's license Strong organizational skills, clinical judgment, and a compassionate communication style Ready to Connect?
