We partner with trusted local home health agencies to support clinicians and deliver high-quality, patient-centered care in the home. We are seeking a Home Health Occupational Therapist (OT) who values flexibility, efficient workflows, and a supportive team that understands what home health is actually like. Why Work With TheraLinks Flexible scheduling: You choose when and how much you work Choose your coverage area: We prioritize referrals near you to reduce drive time Fast, efficient documentation: User-friendly EMR designed to keep charting quick and manageable Minimal administrative friction: Clear communication and support when you need it Therapist-owned support: Work with clinicians who understand home health realities Reliable, on-time pay: Competitive per-visit rates with electronic payment Duties Evaluate patients' functional abilities and develop individualized plans of care focused on ADLs and IADLs Provide skilled occupational therapy interventions to improve safety, mobility, and independence in the home Address areas such as dressing, grooming, toileting, transfers, and functional mobility Assess home environments and recommend adaptive equipment or safety modifications as appropriate Educate patients and caregivers on strategies to promote independence and reduce fall risk Communicate patient progress and clinical changes with the interdisciplinary care team and physician as needed Maintain accurate, timely electronic documentation in compliance with home health and regulatory standards Qualifications Active and unrestricted California Occupational Therapist license Graduate of an accredited Occupational Therapy program Home health experience preferred, but not required Strong understanding of ADLs, functional mobility, and patient safety Excellent communication and time-management skills Ability to work independently in a home-based setting Reliable transportation and valid driver's license
