  • Part-time, Full-time, PRN, Per diem Walnut Creek Skilled Nursing and Rehabilitation Center is in search of an Occupational Therapist to join our team.
We have helped many patients achieve outstanding results, and our staff has experience caring for people recovering from numerous conditions, including cardiac, orthopedic, neurological, and other illnesses . We provide inpatient therapy services seven days a week. Our facility also provides a sub-acute care program for residents recently released from the hospital. Our main goal is to reduce the risk of readmission to the hospital as well as weaning our patients off of tracheostomy and ventilators to get them to a lower level of care. We wish to promote quality of life to each patient. We seek to help our patients in the recuperation process while helping them achieve their maximum capabilities to return home as quickly as possible. Our facility has an outstanding reputation in the healthcare industry with a high rating for quality patient care. Job Description:
The primary role of the Occupational Therapist is to direct all aspects of quality patient care, including evaluation, treatment, treatment planning, goal setting, family education and documentation according to organization's guidelines, professional standards and community needs. This position will also serve as a patient and rehabilitation advocate for current prospective patients, to all internal and external customers while participating in quality assurance and customer service activities.

Qualifications:
Graduate of an approved curriculum in Occupational Therapy, is able to practice unencumbered, is in good standing with all regulatory agencies and licensing boards, has working knowledge of Medicare and other payer sources, full knowledge of residents' rights, exude professionalism in presentation, able to work on multitasks at the same time. In addition, qualified candidates must have proven success in providing extraordinary care, working as a proactive and contributing part of a dynamic team.
