Job Description

Overview Join the dedicated team at Yucaipa Hills Post Acute as an Occupational Therapist, where your skills and compassion will help residents regain independence, improve daily functioning, and enhance their overall quality of life. In this role, you will assess residents' needs, create individualized treatment plans, and provide therapy services that support safe recovery, successful rehabilitation, and continued wellness. As part of our skilled nursing and post-acute care team, you will work closely with nursing, therapy, social services, case management, residents, and families to provide resident-centered care in a supportive and professional environment. Duties Conduct comprehensive evaluations of residents' physical, cognitive, and functional abilities. Develop and implement individualized occupational therapy treatment plans based on resident needs and physician orders. Assist residents with activities of daily living, including dressing, grooming, feeding, toileting, bathing, and mobility-related tasks. Provide therapeutic interventions to improve strength, coordination, balance, endurance, safety awareness, and independence. Educate residents, families, and caregivers on adaptive techniques, therapeutic exercises, assistive devices, and safety measures. Collaborate with the interdisciplinary team, including nursing, physical therapy, speech therapy, social services, dietary, and case management. Participate in care planning, discharge planning, and resident progress reviews. Support residents recovering from illness, injury, hospitalization, surgery, or changes in functional status. Maintain accurate and timely documentation of evaluations, treatment sessions, progress notes, and discharge summaries in accordance with facility policy and regulatory requirements. Ensure therapy services are provided in compliance with federal, state, and facility guidelines. Skills and Qualifications Current Occupational Therapist license in good standing. Experience in a skilled nursing facility, post-acute care, rehabilitation, or geriatric setting preferred. Strong knowledge of anatomy, physiology, rehabilitation techniques, and activities of daily living. Ability to develop individualized treatment plans that support resident independence, safety, and quality of life. Experience working with older adults, residents with cognitive impairments, orthopedic conditions, neurological conditions, and general rehabilitation needs. Strong communication and teamwork skills. Ability to collaborate effectively with residents, families, nursing staff, therapy team members, and facility leadership. Knowledge of documentation standards and healthcare regulatory compliance. $45.00 - $50.00 per hour
