Anova Care, a provider of home care and home health services, is looking for Occupational Therapists (OT), to join our rapidly growing home health program. In this role, OTs will render home health services to patients.Key Responsibilities:
Conduct patient evaluations and develop individualized OT treatment plans Provide skilled occupational therapy interventions and patient education in the home setting Monitor and document patient progress and outcomes in the EMR Communicate effectively with patients, families, and interdisciplinary teams Ensure compliance with home health regulations, physician orders, and documentation standards Participate in quality improvement initiatives and care coordination meetings Collaborate with clinical, administrative, and operations teams to support patient care and service delivery Perform other clinical or administrative duties as assignedPosition Qualifications:
Master's or Doctorate in Occupational Therapy required Current state licensure as an Occupational Therapist 1-3 years clinical experience in home health, outpatient, or rehabilitation settings preferred Strong assessment, treatment planning, and patient education skills Knowledge of home health regulations and documentation requirements Proficiency with EMR systems and Microsoft Office Suite Excellent written and verbal communication and interpersonal skills Ability to work independently and manage a patient caseload Availability during business hours with occasional after-hours/weekend coverageHours:
Monday - Friday, weekends as needed.Work Type:
