Position Summary:

We are seeking an experienced Occupational Therapist to provide in-home therapy services. This role requires a self-motivated professional who can work independently, manage a mobile caseload, and deliver comprehensive patient care from evaluation through treatment planning and documentation.

Key Responsibilities:

Evaluate patients in their homes and develop individualized treatment plans Create and submit physician orders based on clinical findings Provide skilled occupational therapy services to improve patient function and independence Maintain accurate, timely documentation using Axxess charting system Communicate effectively with physicians, care teams, and family members Manage schedule and travel between patient homes efficiently

Qualifications:

Licensed Occupational Therapist in the state of Colorado Minimum of 1-2 years of relevant clinical experience (home health preferred) Strong ability to work independently and manage time effectively Experience with patient evaluations, care planning, and physician communication Proficiency with electronic medical records; Axxess experience preferred Valid driver's license and reliable transportation
