Assess patients' functional abilities and establish goals to maximize independence with activities of daily living (ADLs). Implement and modify treatment plans based on patient progress and changing clinical needs. Complete OASIS assessments and maintain timely, accurate clinical documentation. Educate patients and caregivers on adaptive equipment, home safety, energy conservation, and therapeutic techniques. Fabricate or recommend adaptive equipment and splints as appropriate. Collaborate with physicians, nurses, Certified Occupational Therapy Assistants (COTAs), and the interdisciplinary care team to ensure quality patient outcomes. Supervise COTAs and support continuity of patient care. Ensure compliance with physician orders, agency policies, and home health regulations. Qualifications Graduate of an accredited Occupational Therapy program. Current, unrestricted Connecticut Occupational Therapist license. Minimum of one (1) year of clinical experience. Home health experience is a plus but not required. Valid driver's license, reliable transportation, and current auto insurance. Ability to travel throughout the assigned territory.
