Team That Values Clinical Excellence Responsibilities:
Perform occupational therapy evaluations and reassessments in the home setting Develop and implement individualized treatment plans based on patient needs and physician orders Provide therapeutic interventions focused on improving activities of daily living (ADLs), mobility, strength, coordination, and safety Educate patients, caregivers, and family members on treatment goals and home exercise programs Complete all required documentation accurately and timely Collaborate with physicians, nurses, PTs, and interdisciplinary care team members Maintain compliance with Medicare and agency guidelines
Qualifications:
Current and active Florida Occupational Therapist (OT) license Minimum of one (1) year of Home Health experience preferred Medicare Home Health experience strongly preferred Experience with EMR systems (WellSky/Kinnser preferred) Strong clinical assessment and communication skills Current CPR certification Valid driver's license and reliable transportation
