TBD Job Summary:
We are seeking a compassionate and skilled Occupational Therapist (OT) to provide home health services to patients in their homes. The Occupational Therapist will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to improve independence and quality of life.
Key Responsibilities:
Evaluate patients and develop individualized treatment plans Provide occupational therapy services according to the established Plan of Care Assess patient progress and modify treatment plans as needed Document evaluations, treatments, and patient outcomes accurately Educate patients and caregivers on adaptive equipment, home safety, and therapeutic exercises Collaborate with physicians and the interdisciplinary care team Maintain compliance with all state, federal, Medicare, and agency regulations
Qualifications:
Active Florida license as an Occupational Therapist (OT) Minimum of 1 year of home health experience preferred Strong clinical assessment, documentation, and communication skills Ability to work independently and manage a flexible schedule Reliable transportation and a valid driver's license
Preferred Skills:
Experience with electronic medical records (EMR) Knowledge of Medicare guidelines and home health documentation standards Bilingual (English/Spanish) is a plus
