Duties Conduct thorough assessments of patients' health conditions and needs. Develop and implement individualized care plans based on patient assessments. Collaborate with clinical managers and referral sources to build appropriate plans of care. Monitor clinical performance metrics to optimize patient outcomes. Complete daily workflow Comply with state and federal regulatory guidelines. Work in a team environment with operational support and sales team. Assist with weekend assessments, on rotation basis. Provide other duties as needed. Skills The successful candidate will demonstrate the following skills: Clinical care delivery EMR proficiency Home healthcare regulations Growth-minded Works well with sales team Team player Ability to lift >20lbs, manage patient tasks in the home setting Ability to assess and monitor vital signs accurately. Familiarity with evidence-based practice We encourage clinicians to own their craft of the healing process, while maintaining compliance, positive energy, and teamwork for the growth of Bravery and their profession. Creative use of velcro, fabrication of splints, and a few cognitive tests doesn't hurt either.
