Responsibilities Conduct comprehensive occupational therapy assessments to evaluate patients' functional abilities and needs Develop and implement individualized treatment plans aligned with clinical goals and patient-centered outcomes Assist patients with activities of daily living (ADLs) such as dressing, grooming, feeding, and mobility to promote independence Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless transitions across care settings Manage electronic health records (EHR) accurately, documenting clinical notes, progress reports, and treatment outcomes Provide education and training to patients, families, and caregivers on therapy techniques and adaptive strategies Skills Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards Experience working with individuals with disabilities in skilled nursing facilities. Proficiency in occupational therapy assessments and treatment plan development for pediatrics, geriatrics, and adult populations Skilled in assisting with ADLs and implementing occupational therapy programs tailored to specific needs such as post-acute care or outpatient rehabilitation Familiarity with case management practices including discharge planning and client communication Ability to collaborate effectively within interprofessional teams including speech therapists and other healthcare professionals Join us to empower individuals through compassionate care and innovative therapy solutions.
