Job Description

Come join our wonderful team at Destination. Position Overview Provides quality rehabilitation care in Occupational Therapy to residents as outlined in the plan of care in accordance with laws, regulations and Facility guidelines.
Education & Certification:
Graduate of an accredited occupational therapy program (BS or MS-Occupational Therapy)
Experience:
Must have a current OT license for state of practice in good standing; must be NBCOT certified; one year experience in a health care setting; long-term care experience preferred
Rate:
Commensurate with experience Job Duties Assesses functional levels and capabilities of residents. Conducts ongoing assessments of resident needs and makes recommendations related to resident therapy treatments. Provides occupational therapy treatments by following the resident plan of care. Develops goals for activities of daily living which are measurable and realistic, based on resident's care plan. Participates in the discharge planning process for residents. Thoroughly documents treatment progress in resident charts in an accurate and timely manner. Maintains a positive working relationship with residents and families. Documents appropriately and timely in accordance with laws, regulations and Facility guidelines. Performs overall supervision to OTAs and rehabilitation aides to ensure quality rehab care services are provided. Completes required company forms and documents to ensure compliance with state and federal regulations.
