This dynamic position offers the opportunity to deliver personalized, evidence-based occupational therapy services across diverse populations, including geriatrics, and individuals with neurodevelopmental disorders. As a key member of our interdisciplinary team, you will develop and implement tailored treatment plans that promote functional recovery and support clients' unique needs in a home setting. Responsibilities Conduct comprehensive occupational therapy assessments to evaluate patients' functional abilities and develop individualized care plans. Implement evidence-based occupational therapy programs focused on improving activities of daily living (ADLs), mobility, and overall independence. Coordinate discharge planning and collaborate with case management teams to ensure smooth transitions from hospital or outpatient settings to home care. Document clinical findings, treatment progress, and care plans accurately using electronic health records (EHR) systems while maintaining compliance with medical documentation standards. Support client communication by educating patients and families about therapy goals, progress, and strategies for ongoing care at home or in community settings. Skills Proven experience working with individuals with disabilities, neurodevelopmental disorders, or special education needs across various settings such as outpatient clinics, skilled nursing facilities, or inpatient units. Strong knowledge of anatomy, physiology, medical terminology, and treatment plan development tailored to diverse patient populations. Skilled in implementing occupational therapy assessments and developing customized treatment plans that align with evidence-based practices. Excellent communication skills for collaborating with interprofessional teams including speech therapists, physical therapists, physicians, educators, and caregivers. Experience working with clients on discharge planning, case management, and post-acute care transitions within home health environments. Familiarity with electronic health records (EHR) management and clinical documentation standards to ensure accurate recordkeeping.
