$57.00 per hour About the Role Breezy Hills Rehabilitation and Care Center is seeking a compassionate and experienced PRN Occupational Therapist (OT) to join our rehabilitation team. The OT will provide skilled therapy services to residents in a Skilled Nursing Facility (SNF) setting, helping promote independence, mobility, safety, and quality of life. Responsibilities Provide individualized occupational therapy evaluations and treatment plans. Participate in resident assessments, care planning, and discharge planning. Deliver skilled OT services consistent with resident needs and physician orders. Maintain accurate therapy documentation, progress notes, and medical records. Collaborate with nursing, rehabilitation, administration, and other interdisciplinary team members. Support quality assurance, regulatory compliance, and best practices in rehabilitation services. Communicate effectively with residents, families, and the interdisciplinary care team. Qualifications Active Florida Occupational Therapist (OT) license. Minimum 3 years of OT experience in a Skilled Nursing Facility (SNF) preferred. Strong knowledge of long-term care, rehabilitation, and skilled nursing practices.
