Job Description

Tender Touch Rehab Services seeks a compassionate Occupational Therapist to provide high-quality, evidence-based care to seniors in skilled nursing and long-term care settings. In this onsite role, you will assess functional abilities, develop individualized treatment plans, and deliver interventions focused on ADLs, mobility, cognition, and safety to maximize independence and quality of life. You'll collaborate closely with an interdisciplinary team, educate patients and families, and document care in our EMR. Responsibilities Evaluate senior patients in skilled nursing and long-term care settings to identify functional strengths and limitations Develop and implement individualized, evidence-based OT care plans to improve independence and quality of life Provide therapeutic interventions including ADL training, functional mobility, cognitive retraining, and adaptive equipment use Document evaluations, treatment sessions, and progress notes accurately in the EMR system Collaborate with an interdisciplinary team, including PT, SLP, nursing, and physicians, to coordinate patient-centered care Educate patients, families, and caregivers on home programs, safety, and functional strategies Participate in care conferences and contribute to discharge planning and recommendations Support a compassionate, dignified, and respectful environment for seniors aligned with Tender Touch Rehab Services' values Engage in professional development, mentorship, and clinical training opportunities Adhere to regulatory, ethical, and company standards of clinical practice Required Skills Geriatric occupational therapy
