Job Description

About the Role Hometown Home Health is seeking a licensed Occupational Therapist to provide in-home therapy services to patients throughout the Jacksonville area. This role is ideal for a clinician who enjoys working independently, building strong patient relationships, and helping individuals regain the skills and confidence needed to function safely in their daily lives. About Hometown Home Health Hometown Home Health is a full-service home health agency serving patients throughout Greater Jacksonville and St. Augustine. Our clinical team provides skilled nursing, therapy, rehabilitation, and other medical services directly in the home, with care plans tailored to each patient's specific needs and goals. What You'll Be Responsible For Complete occupational therapy evaluations and reassessments in patients' homes Develop individualized treatment plans based on functional needs and physician orders Provide therapeutic interventions focused on mobility, self-care, safety, and activities of daily living Educate patients and caregivers on exercises, adaptive strategies, and safe techniques Collaborate with physicians and other members of the home health care team Monitor patient progress and recommend changes to the plan of care as needed Complete visit documentation and progress notes accurately and on time Qualifications Graduate of an accredited Occupational Therapy program Current Occupational Therapist license in the state of Florida Current CPR certification; online-only certification is not accepted At least one year of relevant clinical or institutional experience AOTA membership preferred Reliable transportation and ability to travel to patient homes Strong communication, organization, and patient-care skills Why Join Hometown Home Health?
