Position Overview We are seeking an experienced and compassionate PRN Occupational Therapist (OT) to join our growing Home Health team. In this flexible, field-based role, you'll help patients regain independence and achieve their functional goals—all from the comfort of their homes. You'll collaborate with our interdisciplinary team to develop individualized care plans that focus on restoring physical ability, promoting safety, and improving overall quality of life. Key Responsibilities Provide skilled occupational therapy services to home health patients as prescribed by the physician. Evaluate each patient's functional status and establish measurable, achievable goals for improvement. Develop and implement personalized treatment plans to help patients regain mobility, independence, and daily living skills. Reassess patient progress regularly and adjust treatment plans as necessary. Educate patients, caregivers, and families on adaptive techniques and use of assistive equipment. Collaborate closely with the interdisciplinary care team to ensure coordinated, holistic patient care. Maintain timely and accurate clinical documentation in compliance with state, federal, and Nayar Health Care standards. Qualifications Graduate of an accredited Occupational Therapy program approved by the American Occupational Therapy Association (AOTA). Active Occupational Therapist (OT) license in the state of Florida. Minimum of 1 year of experience in a clinical or institutional setting (home health experience preferred). Current CPR certification (required). Clear background check and compliance with all state and federal regulations. Membership in the AOTA preferred.
