Job Description

We are seeking a compassionate and experienced Occupational Therapist (OT) to provide skilled therapy services to patients in their homes. The OT will evaluate patients, develop individualized treatment plans, and provide interventions focused on improving independence, safety, and the ability to perform daily activities. Responsibilities Perform initial and ongoing occupational therapy evaluations. Develop and implement individualized treatment plans in accordance with the physician-approved Plan of Care. Provide skilled occupational therapy treatments to improve strength, coordination, mobility, safety, and activities of daily living (ADLs). Educate patients, families, and caregivers on therapeutic techniques, home exercises, adaptive equipment, and safety precautions. Monitor patient progress and modify treatment plans as appropriate. Communicate changes in patient condition to physicians, clinical leadership, and members of the interdisciplinary care team. Coordinate care with nurses, physical therapists, speech therapists, home health aides, and other healthcare professionals. Complete all required clinical documentation accurately and within required timeframes. Provide supervision to Occupational Therapy Assistants (OTAs/COTAs), when applicable. Maintain compliance with HIPAA, infection control, safety standards, and applicable home health regulations. Qualifications Current and unrestricted Florida Occupational Therapist license . Graduate of an accredited Occupational Therapy program. Current CPR/BLS certification. Home health experience preferred. Strong clinical assessment, communication, and documentation skills. Ability to work independently and manage a field-based patient schedule. Valid driver's license, reliable transportation, and current auto insurance. Must meet applicable background screening and agency credentialing requirements. Position Requirements This is a field-based home health position requiring travel to patients' homes within the assigned service area. The ideal candidate is dependable, compassionate, organized, and committed to helping patients improve their independence and quality of life in the comfort of their homes.
