Job Summary:

We are seeking an experienced Occupational Therapist (OT) for a Home Health contract assignment with BayCare Health System in Sarasota, FL. The ideal candidate must have prior home health experience and be comfortable working independently while providing high-quality patient-centered care in home settings. The OT will evaluate and treat adult and geriatric patients with a variety of orthopedic, neurological, cardiac, and post-surgical conditions while collaborating closely with interdisciplinary healthcare teams.

Key Responsibilities:

Perform patient evaluations, reassessments, discharge planning, and develop individualized treatment plans Provide therapy services for patients with orthopedic, neurological, cardiac, and post-surgical conditions Assist patients with Activities of Daily Living (ADLs), mobility, safety, and functional independence Educate patients and families regarding home exercise programs, safety, and injury prevention Perform home assessments and recommend adaptive equipment as needed Complete accurate documentation, assessments, OASIS documentation, and plan of care updates Utilize modalities and therapeutic interventions including splinting, TENS, ultrasound, edema massage, and joint mobilization Coordinate care with Physical Therapy, Social Services, Pharmacy, and other interdisciplinary teams Maintain productivity expectations of a minimum of 6 points/visits per day

Required Qualifications:

Active FL OT License BLS Certification required Minimum 1 year of OT experience required (2 years preferred) Previous Home Health experience required Must be an independent practitioner Valid driver's license and reliable transportation required for home visits Strong documentation and patient education skills

Preferred Qualifications:

OASIS trained preferred Experience with prosthetics/orthotics and standardized assessments preferred Experience with orthopedic, neurological, cardiac, and geriatric patients preferred

Patient Population:

Adults and Geriatrics Diagnoses include: COPD, CHF, CVA, Parkinson's, trauma, weakness, falls, orthopedic injuries, post-surgical recovery, neuro conditions, respiratory patients, AKA/BKA, and ADL deficits
