Role Overview:

As a Full-Time Occupational Therapist in Home Health , you will assess and evaluate patients' functional status, activities of daily living, fine motor skills, and adaptive equipment needs. You will develop and implement personalized therapy plans in collaboration with physicians, using evidence-based techniques to improve patient outcomes.

Key Responsibilities:

Provide services within the scope of practice according to state laws and the patient's plan of care Evaluate patients' functional abilities and monitor progress, documenting changes and responses to treatment Conduct assessments to determine functional levels using diagnostic tests Communicate observations and patient needs to the healthcare team Perform home assessments, ensuring care adapts to diverse environments and patient backgrounds

Qualifications and Skills:

Current Occupational Therapy license in the practice state Valid driver's license and access to reliable transportation

Preferred:

CPR certification, strong multitasking, independent working skills, and excellent communication and organizational abilities
