The successful candidate will be responsible for providing quality care to patients in their homes, assessing and monitoring patient conditions, and providing education and support to patients and their families. The ideal candidate for this position will have excellent communication skills, an ability to work independently, and a commitment to providing the highest level of care. We are seeking a full-time Occupational Therapists to join or our growing team to service any of our multiple territories in the
Western Suburbs Requirements:
Bachelor's degree from an accredited Occupational Therapy program Active Occupational Therapy licensure in the state of Illinois Possess a valid driver's license and current auto insurance Previous experience in home care setting is desired In-depth, up-to-date knowledge of occupational therapy, theory and practice and the legal and ethical issues associated with them
Essential Functions:
As an Occupational Therapist, you will provide skilled therapy intervention and treatment to patients in the home setting. This care includes appropriate utilization of treatment plans and assessment skills. Careful monitoring of the patient's status and progress toward goals as well as compliance with physician's orders. Responsible for educating patients and caregivers while preparing the patient/caregiver for discharge as appropriate. Implement occupational therapy services in accordance with physician's plan of treatment. Assist the physician in the evaluation and assessment of the client's level of functioning by applying appropriate tests and assist in the development and revision of the plan of care. Promote coordination of care through interdisciplinary case conference and communication with physician and field staff. Initiate diagnostic, preventive and rehabilitative procedures as appropriate to the client's care.
