Colonial Nursing and Rehab - Crown Point, IN Colonial Nursing and Rehab is seeking a compassionate and dedicated Occupational Therapist (OT) to join our skilled nursing and rehabilitation facility in Crown Point, IN. In this role, you will deliver high-quality therapy services designed to improve residents' independence, function, and overall quality of life. Position Details Part-Time What You'll Do Implement individualized treatment plans developed in collaboration with the rehabilitation team Conduct thorough evaluations and establish measurable short- and long-term therapy goals Educate residents, families, and caregivers on techniques that support ongoing progress Work closely with interdisciplinary team members to coordinate comprehensive care Monitor resident progress and adjust treatment plans as appropriate Maintain accurate and timely clinical documentation What You'll Bring Active Occupational Therapist license (OTR/L) in Indiana or associate's degree from an accredited OT program Commitment to patient-centered care Strong communication and interpersonal skills Ability to work effectively as part of an interdisciplinary team Previous skilled nursing or rehabilitation experience preferred Why Join Our Team?
