Job Description

Franciscan Health Dyer Campus 24 Joliet St Dyer, Indiana 46311 The Occupational Therapist I provides evaluation and treatment of the patient, within the parameters of legal and professional guidelines, and hospital and departmental policies and procedures. The position instructs and directs Occupational Therapist Assistant and aides as assigned. The Occupational Therapist evaluates and implements the plan of care to improve, restore, and enhance performance in activities of daily living and functional mobility. This position performs documentation and quality improvement as outlined in departmental policies and procedures.

WHAT YOU CAN EXPECT
Complete appropriate goal re-assessment and discharge planning. Provide occupational therapy, according to standards of care defined by the state practice act and other professional and governing agencies. Perform comprehensive occupational therapy evaluations, treatment and related follow-up services, including discharge planning. Document and submit patient reports and charges within the established timeframe. Collaborate with patient and other healthcare personnel or outside agencies regarding coordination of care. Develop a treatment plan with measurable goals based on patient assessment. Provide occupational therapy, including selection and use of assistive equipment. Train and educate patients and family members or other support, and document understanding of treatment plan and goals. Qualifications Required Bachelor's Degree Occupational Therapist Required Master's Degree Occupational Therapist Preferred Doctorate Occupational Therapist None Occupational Therapy Required 1 year Occupational Therapy Preferred Occupational Therapist (OT) - State Licensing Board Indiana or Illinois Cardiopulmonary Resuscitation (CPR) - American Heart Association
